Case Management

At USA Family Well Being Services, we have a client-centered case management service that enhances clients’ functioning in their homes, community and schools. Our highly-skilled, multi-lingual and culturally competent case managers work with children and adults who have experienced emotional or behavioral difficulties due to trauma history, transitions, and cultural adjustment. Each client is assigned a culturally competent case manager who is college-educated and understands mental health and developmental issues. The case manager will make the client’s individual needs and interests the top priority. Our goal is to help our clients gain a high level of independence and experience meaningful personal growth. Many immigrants and refugees have experienced traumatic events in their home countries and in refugee camps. Here in the US, many face issues in trying to adjust to the American way of life.

Case Management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s needs. Our case managers work with clients to advocate for their educational, legal, housing, employment, healthcare, and mental health needs. They help clients and their families access resources available in the community and encourage them towards a higher level of independence.


Case Management for Adults

Our adult case managers work with adults to help in locating resources, becoming independent, and overcoming the challenges of living in their new community. Services include:

 

    • ~ Supporting the client emotion-ally as he or she begins making choices and meeting personal goals.
    • ~ Providing information on employment, housing, recreational/leisure opportunities that will help them become thriving members of their community.
    • ~ Assisting the client in making important decision based upon the client’s strengths and wishes.
    • ~ Assessing clients’ needs and developing an individual service plan with the client.
    • ~ Helping the client find medical, social, educational, and other services needed.
    • ~ Bridging cultural barriers to help the client connect with natural and community supports.
